WHY DO I NEED TO BRING MY INSURANCE CARD- WHEN YOU DO NOT ACCEPT INSURANCE?

If you are given a prescription, your health insurance will oftentimes require prior authorization. Therefore, we must have your insurance information.

CAN I ASK MY INSURANCE COMPANY TO REIMBURSE ME FOR MY APPOINTMENT WITH UPSTATE ADHD CENTER,LLC?

We will gladly provide you with a receipt and you can submit the receipt to your insurance plan for possible reimbursement. There are NO GUARANTEES that your insurance plan will reimburse you. Unfortunately, if you have Medicare, federal law prevents us from treating you at Upstate ADHD Center, LLC even if you want to self pay.

WHAT IS YOUR NEW PATIENT PROCEDURE?

Your first appointment (initial comprehensive psychiatric evaluation) will be online with our psychiatric provider. We do not write prescriptions during this initial evaluation. Cost: $150.

Initial follow-up appointment will be in office. This appointment includes computerized ADHD testing, complete screening tools (if not already completed), meet with psychiatric provider to review your test results, and develop/review your ADHD treatment plan. If prescription medication is recommended as part of your treatment plan, your prescription will be sent to your pharmacy electronically. Cost: $200

Subsequent follow-up (in office) appointments with psychiatric provider to assess how you are responding to your ADHD treatment and to refill and send your prescription electronically to your pharmacy (if applicable). Cost:  $75


HOW LONG ARE THE APPOINTMENT TIMES (AND COST)?

Initial Comprehensive Psychiatric Evaluation (Online): Typically 30 minutes to 1 hour. Cost: $150


Initial Follow-Up Appointment (In Office): Typically 45 minutes to 1 hour. Cost: $200  (includes computerized objective testing)

Subsequent Follow-Up Appointments (In office): Typically 15-20 minutes. Cost: $75
